Our client, a Healthcare company, is looking for a LPN: Home Health for their Fort Wayne, IN location.
Responsibilities:
The Licensed Practical Nurse is a licensed professional who provides direct and indirect patient care services to patients in their home environment or alternative setting.
The LPN collaborates with the Registered Nurse to provide nursing treatments to patients according to the plan of care established.
The LPN provides technical nursing interventions including infusion, wound care, phlebotomy, ostomy, catheter care, and other skilled interventions as ordered by the physician and within their Scope of Practice.
The LPN instructs, motivates, and assists patient and/or caregivers in learning and improving ability to live independently at home.
Requirements:
License Requirements: LPN
Productivity Requirement: 30 points per week
Must be comfortable with basic nursing skills such as venipuncture, wound care, disease management teaching.
Preferred experience with HCHB/Point care in Home Health.
