Responsibilities:
- The Licensed Practical Nurse is a licensed professional who provides direct and indirect patient care services to patients in their home environment or alternative setting.
- The LPN collaborates with the Registered Nurse to provide nursing treatments to patients according to the plan of care established.
- The LPN provides technical nursing interventions including infusion, wound care, phlebotomy, ostomy, catheter care, and other skilled interventions as ordered by the physician and within their Scope of Practice.
- The LPN instructs, motivates, and assists patient and/or caregivers in learning and improving ability to live independently at home.
Requirements:
- License Requirements: LPN
- Productivity Requirement: 30 points per week
- Must be comfortable with basic nursing skills such as venipuncture, wound care, disease management teaching.
- Preferred experience with HCHB/Point care in Home Health.
