The Opportunity

If you're looking to practice nursing outside the walls of a hospital or facility, this Home Health LPN role in Medford offers exactly that. You'll deliver skilled nursing care directly in patients' homes, building the kind of one-on-one relationships that are hard to find in higher-volume clinical settings.

Why You'll Love This Role

You'll provide hands-on skilled care, including wound care, medication administration, injections, IV therapy, vital signs monitoring, and patient education, all under RN supervision and a physician-ordered plan of care.

Beyond direct care, you'll take on a leadership role supervising Home Health Aides and providing feedback on their work. You'll also work closely with an RN Case Manager and interdisciplinary team, contributing to care plan development, case conferences, and quality initiatives, so your clinical judgment is valued as part of a bigger picture.

Patient education is central to this role. You'll help patients and families understand disease processes, medications, and safety practices so they can manage care with greater independence, work that offers real, visible impact.

Your Day-to-Day

  • Conduct skilled nursing visits in patients' homes per the physician's plan of care
  • Monitor patient status and report changes promptly to the RN or physician
  • Supervise and guide Home Health Aides on assigned tasks
  • Complete accurate, timely clinical documentation per agency, state, and payer standards
  • Coordinate visit scheduling and travel efficiently across your assigned territory
  • Maintain strict safety and infection control standards during every visit

What We're Looking For

  • One year of clinical experience and one year of home health experience
  • Graduate of an accredited practical or vocational nursing program
  • Current LPN or LVN license in the state of agency operation
  • Valid CPR certification
  • Valid driver's license with ability to travel to all business locations
