What You’ll Do
Under the supervision of a Registered Nurse, you will play a key role in delivering care and supporting positive patient outcomes. Responsibilities include:
Providing direct patient care in accordance with the State Nurse Practice Act
Implementing and following the established plan of care
Accurately documenting care and maintaining timely clinical records
Observing patient conditions and reporting changes to the RN and physician
Collaborating with interdisciplinary team members to coordinate patient services
Communicating effectively with physicians to relay patient needs and obtain orders
Using medical equipment and supplies efficiently and safely
Supporting care coordination by identifying needs for additional services (PT, OT, HHA, etc.)
Participating in professional development and ongoing training
Assisting with on-call responsibilities and holiday rotations as needed
What We’re Looking For
Current Licensed Practical Nurse (LPN) license in the State of Wisconsin
Graduate of an accredited practical nursing program
Minimum of 3 years nursing experience preferred
Home health and/or hospice experience preferred
Strong clinical, communication, and organizational skills
Commitment to professional standards and quality patient care
Valid driver’s license with a reliable, insured vehicle
