    Description

    Monday - Friday, 40 hours / week

    As a Licensed Practical Nurse (LPN) in our Home Health or Hospice program, you’ll provide skilled nursing care to patients in their homes and residential settings under the supervision of a Registered Nurse. You’ll play an important role in helping patients maintain comfort, independence, and quality of life while delivering compassionate, patient-centered care throughout their healthcare journey. 

    What You’ll Do: 

    Provide skilled nursing care, treatments, and medication administration according to physician orders and the plan of care.
    • Monitor patient conditions and communicate changes promptly to the supervising RN.
    • Educate patients and families on diagnoses, medications, treatments, self-care, prevention, and health maintenance.
    • Maintain accurate, timely documentation within the electronic medical record system.
    • Promote a safe home environment and ensure patients have necessary supplies and resources.
    • Collaborate with interdisciplinary team members through case conferences and care planning.
    • Travel within designated service areas to provide patient care visits. • Deliver exceptional customer service while supporting patients and families with compassion and professionalism. 

    Who You Are: 

    • Graduate of a state-approved School of Practical Nursing.
    • Current Licensed Practical Nurse (LPN) licensure in the State of New Hampshire.
    • Minimum of one year of recent nursing experience; home health or hospice experience preferred.
    • Current CPR certification.
    • Valid driver’s license with an acceptable driving record.
    • Strong communication, documentation, and patient education skills.
    • Comfortable working independently while collaborating closely with a multidisciplinary care team.
    • Experience with Allscripts EMR is a plus.
