Assists providers with patient examinations, diagnostic procedures, treatments, and care coordination to facilitate effective delivery of patient care and patient satisfaction. Responsible for the direct provision of ancillary services or advanced, specialized knowledge or experience integral to their job function.
Job Responsibilities
Greet patients and express sensitivity to patient needs and conditions.
Assess and document the general condition of patients, including recording chief complaints, vital signs, weight, medications, social history, and verifying symptoms and interventions in the patient medical record.
Record services and diagnoses accurately for completion of encounter forms, requisitions, consent forms, and advance beneficiary notices.
Assist providers with patient examinations, procedures, and treatments according to established protocols.
Independently perform ancillary services including venipunctures, x-rays, etc., per protocols.
Maintain medical records, including preparation of charts, filing of correspondence, and test results.
Assist in coordination of care through messaging patient telephone calls, scheduling appointments, communicating prescription refills, etc., according to established protocols.
Successfully use IDX, EMR, and other computer applications to accomplish tasks according to policies and procedures.
Track ordered or pending tests, procedures, and referrals according to protocol.
Administer medication orally and via intra-muscular injection as directed by physician order.
Determine eligibility and obtain authorization for services.
Schedule tests and appointments for referred services.
Track referrals made for services.
Promote patient wellness by providing patient education as directed by the provider.
Utilize advanced clinical knowledge or experience to resolve patient questions and meet patient needs.
Participate in quality initiatives and safety functions.
Assist in identifying and ordering needed supplies.
Remain informed and compliant with current health system, OSHA, and HIPAA policies and procedures.
Maintain exemplary patient satisfaction and participate as an engaged team member.
Decision Making: Carry out non-routine procedures under constantly changing conditions, in conformance with general instructions from supervisor.
Working Relationships: Work directly with patients and/or customers.

Required Skills & Experience

Working knowledge of general disease state management processes, bio-psychosocial model, and clinical pathways.
Effective interpersonal, written, and oral communication skills.
Ability to organize and prioritize work in an effective and efficient manner.
Detail-oriented as required in the examination of clinical and numerical data.
Demonstrated patient care skills.
Demonstrated PC skills.
Effective organization of a multi-task role.
Understanding of healthcare delivery processes in the medical office.
Required Education
Completion of the prescribed curricula of a practical nursing program or equivalent coursework as approved and verified by a state board of nursing, holding or entitled to hold a diploma or certificate therefrom.
Required Certification & Licensure
Valid multi-state or State Licensed Practical Nurse License.
