We are currently seeking a LPN Intake Nurse with a can-do attitude and able to perform under pressure. Our ideal candidate will possess excellent communication skills while providing the highest level of customer service to internal and external customers. LPN Intake Summary:

Main Responsibilities:

• Coordinate with clinical manager and physicians offices to ensure proper clinical care of our patients
• Communicate with MD offices with lab results and changes in patient status
• Answers incoming phone calls, gathering and processing patient information from various sources for accurate and comprehensive initial assessment of referrals.
• Obtains necessary referral information to assist with the transition to homecare, including demographics, diagnoses, insurance information, physicians' orders, available caregivers, F2F documentation, etc.
• Assist in the scheduling of patient services
