Title: Nurse-1915(i) RISE

Company/Location: Kentucky River Community Care/All KRCC Counties/Home visits

Credentials: Valid LPN and Board license

EmploymentType: PRN

Special Skills/Knowledge Required: Ability to make home visits; electronic medical record experience; timely completion of documentation; experience with psychotropic medications; excellent communication and interpersonal skills.

Primary Functions of job:

• Medication management for adults with serious mental illness.
• Provide education and training about medications.
• Monitoring medication adherence.
• Timely documentation of services provided.

Shift/Days & Work Hours: Varied days and hours based upon client referrals. Flexible scheduling
