Responsibilities
Provide patient care under supervision, following LPN scope and clinical guidelines.
Document patient status and care plans accurately in electronic health records.
Monitor vital signs and report variances to nursing team promptly.
Assist with medication administration and track dosage accuracy.
Identify and escalate potential patient safety risks in a timely manner.
Collaborate with physicians and RNs to adjust care plans based on progress.
Maintain a clean, safe, and compliant clinical environment.
Engage in continuous learning to improve clinical skills and clinical decision-making.
Support patient and family education on care plans and post-discharge steps.
Participate in on-site shift handoffs and ensure continuity of care.
Assist in routine clinical tasks to improve operational efficiency.
Contribute to performance metrics by meeting individual and team targets.
Experience/Qualifications
Licensed Practical Nurse (LPN) license valid in applicable state.
Proficiency with electronic health records and care plan documentation.
Vital signs monitoring with rapid escalation of variances.
Medication administration support with dosage accuracy and safety checks.
Strong patient safety mindset and escalation of potential risks.
Ability to collaborate with physicians and RNs to adjust care plans.
Maintain clean, safe, and compliant clinical environment per policies.
Commitment to continuous learning and clinical skill development.
Patient and family education on care plans and post-discharge steps.
On-site shift handoffs and continuity of care across team members.
