LPN - Licensed Practical Nurse

Summary: Responsible for providing support and care to meet the physical and psychological needs of consumers through providing direct and indirect care.
Duty or Responsibility Provide supervision and monitoring of each consumer. Assist with and model appropriate daily living skills. Document any reported or observed symptoms of consumers and report to the team. Record bed checks via the Bed Board every 30 minutes or more often if ordered by provider. If asked, document progress notes in NextGen for psychoeducational groups. Maintain patient confidence and protects medical office/unit operations by keeping information confidential;cautioning others regarding potential breaches. Participate in Shift Report All other duties as assigned by supervisor
