Job Summary

The Licensed Practical Nurse LPN provides professional nursing care for residents of all ages in long-term care under the supervision of a registered nurse, advanced practice provider, or physician. The LPN demonstrates knowledge of nursing principles for the resident in the relevant clinical setting. Demonstrates competency and practices within the full scope of nursing expertise, knowledge, and utilizes appropriate age and population-specific standards related to the physical and psychosocial needs of the resident as per care plan.

Responsibilities

  • Provide direct nursing care to residents in a long-term care setting
  • Collaborate with an interdisciplinary care team
  • Support resident-centered care plans
  • Maintain a safe, compassionate, and respectful environment
  • Contributes to the assessment of residents and administers medications or treatments as ordered
  • Provides technical support to healthcare professionals as needed
  • Documents resident interactions as needed and the outcomes related to resident care, medication behavior, activity, and dietary
  • Skilled in establishing and maintaining effective working relationships with resident healthcare providers and the public
  • Ability to communicate clearly and maintain confidentiality of sensitive information
  • Counsels residents, family members, and/or resident advocate under the direction of a registered nurse, advanced practice provider, or physician on prevention and treatment plans
  • Knowledge of resident education principles to develop a self-care program
  • Performs other related duties as assigned
