LPN Responsibilities:

• Provide direct patient care and resident care as a Licensed Practical Nurse

• Administer medications and treatments per physician orders

• Monitor residents’ conditions and report changes to RN or physician

• Accurately document all LPN care provided

• Collaborate with RNs, physicians, and the interdisciplinary team to ensure quality outcomes

LPN Qualifications:

• Active LPN license in Kentucky
