Responsibilities
Provide direct patient care under the supervision of registered nurses.
Administer medications and monitor patient responses.
Participate in ongoing professional development and training.
Assist in developing and implementing patient care plans.
Collaborate with healthcare team members to optimize patient outcomes.
Engage in performance measurement and improvement initiatives.
Maintain accurate patient records and documentation.
Monitor vital signs and report changes to the nursing staff.
Ensure compliance with healthcare regulations and standards.
Educate patients and families on health management and care plans.
Assist with daily living activities for patients as needed.
Foster a positive and supportive environment for patients and staff.
Experience/Qualifications
Valid LPN license in the state of employment.
Commitment to ongoing professional development and training.
Experience in a clinical setting, preferably in patient care.
Ability to collaborate with healthcare team members for optimal outcomes.
Skills in educating patients and families on health management.
Strong communication skills for effective patient and team interactions.
Proficiency in documenting patient care and maintaining records accurately.
Ability to monitor vital signs and report significant changes promptly.
Knowledge of healthcare regulations and compliance standards.
Willingness to assist patients with daily living activities as needed.
