We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our nursing team on a PRN basis. This role offers the opportunity to make a meaningful impact while providing flexible scheduling to meet facility needs.

Job Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to residents, supports rehabilitation goals, and collaborates with physicians, Registered Nurses, and interdisciplinary team members to ensure exceptional resident outcomes. The ideal candidate is compassionate, clinically skilled, and dedicated to delivering quality care in a long-term care and rehabilitation setting.

Responsibilities

• Administer medications and treatments as ordered by physicians.
• Monitor, assess, and document residents' conditions, vital signs, and changes in health status.
• Report significant changes to the supervising RN and healthcare provider.
• Assist residents with activities of daily living (ADLs) as necessary.
• Collaborate with physical, occupational, and speech therapy teams to support resident rehabilitation goals.
• Maintain accurate and timely clinical documentation in compliance with facility policies and state and federal regulations.
• Educate residents and families regarding care plans and health needs.
• Promote a safe, clean environment while following infection prevention and control guidelines.
• Deliver compassionate, resident-centered care that supports dignity and quality of life.

Qualifications

• Current, active Florida Licensed Practical Nurse (LPN) license.
• Strong clinical skills, including medication administration and wound care.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a collaborative, team-oriented environment.
• Compassionate and patient-focused approach to care.
• Current BLS/CPR certification required.
• Long-term care or rehabilitation experience preferred but not required.
