Job Overview
We are seeking an experienced Licensed Practical Nurse (LPN) to provide compassionate and high-quality nursing care to residents in a long-term care setting. The ideal candidate will have strong clinical skills and experience caring for residents with varying medical and nursing needs.
Key Responsibilities
Provide direct nursing care to residents according to individualized care plans.
Administer medications and treatments as ordered by physicians and in accordance with applicable policies.
Monitor vital signs, pain levels, and residents' overall health status.
Observe, document, and report changes in residents' conditions to the RN or physician.
Assist with wound care, dressing changes, and other nursing procedures.
Maintain accurate and timely clinical documentation.
Communicate effectively with residents, families, nurses, physicians, and other members of the healthcare team.
Promote resident safety, comfort, dignity, and quality of care.
Required Qualifications
Minimum 2 years of experience in the LTC specialty.
Current New Hampshire LPN License or valid Compact State LPN License and registration – Required.
Current AHA Basic Life Support (BLS) Certification – Required.
Strong clinical assessment, communication, and documentation skills.
Ability to work independently and effectively in a long-term care environment.
