Job Description: Position Summary: As a Licensed Practical Nurse (LPN) in our long-term care facility, you will play a vital role in ensuring the well-being and comfort of our residents. You will be responsible for providing direct nursing care, administering medications, and collaborating with the interdisciplinary healthcare team to develop and implement individualized care plans. Key Responsibilities: Patient Care: Provide high-quality nursing care to residents with various medical conditions and needs. Monitor and assess residents' health status, documenting observations and changes in condition. Administer medications and treatments as prescribed by physicians. Assist with activities of daily living (ADLs), including bathing, dressing, grooming, and feeding. Implement infection control measures to maintain a safe and healthy environment. Documentation and Reporting: Maintain accurate and up-to-date medical records, including residents' charts and care plans. Report any significant changes in residents' conditions to the charge nurse or physician promptly. Complete incident reports as needed and participate in the review of adverse events. Collaboration: Collaborate with registered nurses (RNs), certified nursing assistants (CNAs), and other healthcare professionals to develop and revise care plans. Attend team meetings and provide input on resident care and treatment plans. Communicate effectively with residents and their families, addressing their questions and concerns with empathy and professionalism. Safety and Compliance: Adhere to all applicable healthcare regulations, including HIPAA and state nursing guidelines. Maintain a clean and organized work environment, following infection control protocols. Participate in ongoing training and education to stay current with nursing practices and technology. Qualifications: Current and valid LPN license in state of practice. Minimum of 1 year of nursing experience, preferably in long-term care. BLS Certification required. Strong communication and interpersonal skills. Compassionate and patient-centered approach to care. Ability to work independently and as part of a collaborative team. Benefits: Competitive salary.
