    Job Summary:
    Tryfacta is seeking a LPN - LTC for our client in Anchorage, AK 99504. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

    Position Title: LPN - LTC
    Location: Anchorage, AK 99504
    Duration: 13 weeks (Possible extension)
    Work Schedule: Days; Variable – Nights 6pm-6am or 6am-6pm

    Responsibilities for this position include, but are not limited to:
    • Provide direct nursing care to residents in a skilled nursing and long-term care setting.
    • Administer prescribed medications and treatments in accordance with physician orders and facility policies.
    • Monitor, assess, and document residents' conditions, reporting any significant changes to the RN or physician.
    • Assist with wound care, IV therapy (if within scope), and other nursing procedures as permitted by state regulations.
    • Collaborate with RNs, CNAs, physicians, therapists, and the interdisciplinary team to develop and implement resident care plans.
    • Maintain accurate and timely documentation in the electronic medical record (EMR).
    • Respond promptly to changes in resident condition and emergency situations.
    • Follow infection prevention, safety, and regulatory guidelines while providing compassionate, resident-centered care.

    • Active Alaska LPN License (or eligibility to obtain one) required.
    • Minimum 1 year of recent Long-Term Care (LTC) or Skilled Nursing Facility (SNF) experience required.
    • Current BLS Certification required.
    • Experience with medication administration, treatments, and resident assessments.
    • Familiarity with EMR systems such as PointClickCare (PCC) or similar preferred.
    • Strong communication, documentation, and teamwork skills.
    • Ability to work 12-hour day (6:00 AM–6:00 PM) and night (6:00 PM–6:00 AM) shifts with flexibility for both as needed.
    • Must be able to work 36 hours per week for a 13-week travel assignment.
    • Previous travel experience is preferred but not required.
