Job Title: Licensed Practical Nurse (LPN) – Long-Term Care (LTC)
Position Summary

The Licensed Practical Nurse (LPN) in Long-Term Care provides direct nursing care to residents under the supervision of a Registered Nurse (RN) or physician. The LPN ensures residents receive safe, compassionate, and high-quality care in accordance with facility policies, state regulations, and nursing standards.

Key Responsibilities

Provide direct patient care including medication administration, treatments, and wound care

Monitor residents' health status and report changes to the RN or physician

Assist with resident assessments and care plan implementation

Accurately document nursing care, observations, and resident responses

Supervise and support Certified Nursing Assistants (CNAs)

Respond promptly to resident needs and emergencies

Maintain compliance with infection control and safety protocols

Communicate effectively with residents, families, and interdisciplinary teams

Promote resident dignity, comfort, and quality of life

Qualifications & Requirements

Active LPN/LVN license in the state of practice

Graduation from an accredited Practical Nursing program

1 year of Long-Term Care or Skilled Nursing experience preferred

Current BLS/CPR certification

Strong clinical, organizational, and communication skills

Ability to work independently and as part of a healthcare team

Work Schedule

Flexible shifts available (Days / Evenings / Nights)

Weekends and holidays as required

Physical Requirements

Ability to stand, walk, lift, and assist residents as needed

Must be able to perform job duties in a fast-paced healthcare environment
