Job Summary:
Tryfacta is seeking a LPN - LTC for our client in Burlington, VT, 05401. This is a 13-week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: LPN - LTC
Location: Burlington, VT, 05401
Duration: 13 weeks
Work Schedule: ASAP (Tuesday Start Only)

Responsibilities for this position include, but are not limited to:
" Provide direct nursing care to residents in a Long-Term Care/Rehabilitation setting.
" Administer medications and treatments as prescribed.
" Monitor, assess, and document resident conditions using PointClickCare (PCC).
" Collaborate with the interdisciplinary team to ensure quality resident care.
" Assist with admissions, discharges, and care planning.
" Maintain compliance with facility policies and nursing standards.

To be considered for this position, you should have: [Skills, Education, or Experience]
" Current and valid Vermont LPN License (or eligibility to practice in Vermont).
" BLS Certification required.
" Previous LTC/SNF/Rehabilitation experience preferred.
" Experience with PointClickCare (PCC) is a plus.
" Strong communication, teamwork, and patient care skills.
