Licensed Practical Nurse - Long Term Care
Job Summary
Delivers nursing care to residents in a long-term care setting under the direction of a Registered Nurse or physician. Administers medications and treatments, monitors resident status, and supervises the work of Certified Nursing Assistants. Maintains accurate clinical documentation and communicates changes in resident condition to the care team.
Education & Experience
• Graduate of an accredited practical or vocational nursing program: Required
• Prior experience in a long-term care or skilled nursing setting: Preferred
Licenses & Certifications
• Licensed Practical Nurse or Licensed Vocational Nurse licensure in the state of practice: Required
• Basic Life Support (BLS) certification: Required
Key Responsibilities
• Administer medications and treatments in accordance with physician orders and facility policy.
• Perform resident assessments and monitor for changes in physical, cognitive or behavioral status.
• Supervise and delegate appropriately to Certified Nursing Assistants on the assigned unit.
• Complete accurate and timely documentation in the resident record and medication administration record.
• Communicate changes in resident condition to the Registered Nurse, physician and family as appropriate.
• Follow infection control, safety and regulatory requirements governing long-term care practice.
