Job Summary:
Tryfacta is seeking aLPN (LTC) for our client in Eagle Butte, SD, 57625. This is a 13 weeks contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title:LPN (LTC)
Location: Eagle Butte, SD, 57625
Duration: 13 weeks
Work Schedule: Variable Days | 10:00 AM – 10:30 PM (12-Hour Shifts)

Job Responsibilities:
  • Provide direct nursing care to residents in a long-term care setting.
  • Administer medications and treatments as prescribed.
  • Monitor, assess, and document residents' conditions and report changes to the healthcare team.
  • Assist with admissions, discharges, and care planning.
  • Collaborate with physicians, RNs, CNAs, and interdisciplinary team members to ensure quality patient care.
  • Maintain accurate and timely documentation in accordance with facility policies.
  • Follow infection prevention and control protocols.
  • Respond promptly to resident needs and medical emergencies while promoting a safe and compassionate environment.

Requirements:
  • Active LPN license
  • Long-Term Care experience preferred
  • Current BLS certification
  • Ability to work independently in a rural healthcare setting
  • Strong clinical, communication, and documentation skills
