Position Title: LPN LTC Location: Green Bay, WI Duration: 13 weeks Work Schedule: 8/16 hrs. shift
Pay Rate: 45-50$/hr.
Responsibilities
Provide direct nursing care to residents in a Long-Term Care (LTC) setting.
Administer medications and treatments as prescribed by physicians.
Assess, monitor, and document residents' conditions, reporting any changes promptly.
Develop and implement individualized care plans in collaboration with the healthcare team.
Maintain accurate and timely clinical documentation.
Ensure compliance with infection control, patient safety, and facility policies.
Educate residents and families on medications, treatments, and health management.
Deliver compassionate, resident-centered care while promoting quality outcomes.
