Responsibilities

  • Provide licensed practical nursing care in long-term care settings under supervision.

  • Monitor residents' health status and report changes to the supervising nurse.

  • Administer medications and document administration accurately in charts.

  • Maintain rigorous accuracy and timeliness in documentation and reporting.

  • Uphold safety and infection control standards across all tasks.

  • Assist with activities of daily living to support resident independence.

  • Collaborate with care team to develop and adjust individualized care plans.

  • Demonstrate a growth mindset by pursuing ongoing training and certification.

  • Engage in performance-driven activities with clear metrics and targets.

  • Participate in team huddles and report progress toward outcomes and pipelines.

  • Assist with care transitions and family communications as needed.

  • Maintain on-site presence and punctuality to support resident care continuity.


Experience/Qualifications

  • Licensed Practical Nurse (LPN) license in good standing.

  • Experience in long-term care or geriatric patient care settings.

  • Accurate medication administration with charting in electronic health records.

  • Willingness to pursue ongoing training and certifications (growth mindset).

  • Goal-driven performance with clear metrics and accountability.

  • Strong documentation discipline with timely, comprehensive chart updates.

  • Vaccinations and infection control standards adherence.

  • Collaborative care planning with interdisciplinary team.

  • Ability to assist with activities of daily living and resident independence.

  • On-site presence, punctuality, and reliability to support continuity of care.
