LPN LTC

Job Summary:

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care (LTC) team. The LPN will provide direct patient care, monitor health conditions, and support residents' daily needs while working under the supervision of Registered Nurses (RNs) and physicians.

Key Responsibilities:

• Provide direct nursing care to residents in accordance with care plans
• Administer medications and treatments as prescribed
• Monitor and record patient vital signs and health status
• Assist residents with daily living activities (ADLs) such as bathing, dressing, and mobility
• Observe and report changes in patient condition to the RN or physician
• Maintain accurate and timely documentation of patient care
• Ensure compliance with healthcare regulations and facility policies
• Support infection control practices and patient safety standards
• Collaborate with interdisciplinary teams to provide quality care

Requirements:

• Active LPN/LVN license in the state of practice
• Minimum 1 year of experience in Long-Term Care (preferred)
• Basic Life Support (BLS) certification required
• Strong clinical and patient care skills
• Excellent communication and documentation skills
• Ability to work in a fast-paced environment

Shift & Schedule:

• Day / Night shifts available
• Weekends and holidays as required
