Job Summary:
Tryfacta is seeking a LPN LTC for our client in Palmyra, MO 63461. This is a 13 week contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: LPN LTC
Location: Palmyra, MO 63461
Duration: 13 weeks (Possible extension)
Work Schedule: 8 hours Evening with every other weekend.

Responsibilities for this position include, but are not limited to:
  • Provide direct nursing care to residents in a long-term care setting.
  • Administer prescribed medications, treatments, and injections safely and accurately.
  • Monitor, assess, and document residents' health conditions and report changes to the RN or physician.
  • Assist with developing and implementing individualized care plans.
  • Perform wound care, dressing changes, catheter care, and other nursing procedures within LPN scope of practice.
  • Maintain accurate and timely documentation in the electronic medical record (EMR).
  • Collaborate with physicians, RNs, CNAs, therapists, and other healthcare professionals to ensure quality patient care.
  • Supervise and delegate appropriate tasks to Certified Nursing Assistants (CNAs).
  • Respond promptly to changes in resident conditions and medical emergencies.
  • Educate residents and their families on treatments, medications, and ongoing care needs.
  • Follow all facility policies, infection control guidelines, and safety regulations.
  • Maintain a clean, safe, and comfortable environment for residents while promoting dignity and independence.

  • Active LPN License
  • LTC/SNF/Nursing Home experience preferred
  • Medication administration experience
  • EMR documentation skills
  • BLS/CPR certification
  • Ability to work evening shifts and every other weekend
  • Strong patient care and communication skills
