Responsibilities
Provide skilled nursing care to LTC residents under licensed supervision.
Assess resident conditions, document changes, and communicate to care team.
Administer medications and treatments per physician orders and policies.
Develop and update individualized care plans with measurable goals.
Monitor and report adverse events, participating in quality improvement initiatives.
Maintain accurate patient records and ensure HIPAA/compliance standards.
Demonstrate accountability for own outcomes within a team-driven, target-focused environment.
Collaborate with interdisciplinary team to ensure seamless resident care.
Provide resident and family education to support ongoing care at home.
Demonstrate professional growth by pursuing certifications andOn-the-job learning.
Utilize clinical pipelines and metrics to track acuity, bed occupancy, and care milestones.
Experience/Qualifications
Licensed Practical Nurse (LPN) with active state license required.
Experience in long-term care (LTC) or skilled nursing environment preferred.
Administer medications and treatments per physician orders and policies.
Accurate documentation and HIPAA/compliance adherence at all times.
Develop and update individualized care plans with measurable goals.
Ability to assess conditions, document changes, and escalate appropriately.
Collaborate with interdisciplinary team and coordinate resident care.
