Looking for a night nurse every Thursday, Saturday, Sunday, weekend warrior,

36hrs, Variable - Nights, 05/18


Responsibilities

  • Provide skilled nursing care to LTC residents under licensed supervision.

  • Assess resident conditions, document changes, and communicate to care team.

  • Administer medications and treatments per physician orders and policies.

  • Develop and update individualized care plans with measurable goals.

  • Monitor and report adverse events, participating in quality improvement initiatives.

  • Maintain accurate patient records and ensure HIPAA/compliance standards.

  • Demonstrate accountability for own outcomes within a team-driven, target-focused environment.

  • Collaborate with interdisciplinary team to ensure seamless resident care.

  • Provide resident and family education to support ongoing care at home.

  • Demonstrate professional growth by pursuing certifications andOn-the-job learning.

  • Utilize clinical pipelines and metrics to track acuity, bed occupancy, and care milestones.


Experience/Qualifications

  • Licensed Practical Nurse (LPN) with active state license required.

  • Experience in long-term care (LTC) or skilled nursing environment preferred.

  • Administer medications and treatments per physician orders and policies.

  • Accurate documentation and HIPAA/compliance adherence at all times.

  • Develop and update individualized care plans with measurable goals.

  • Ability to assess conditions, document changes, and escalate appropriately.

  • Collaborate with interdisciplinary team and coordinate resident care.

  • Utilize clinical pipelines and metrics to track acuity, bed occupancy, and milestones.

  • Demonstrate accountability for own outcomes within a target-driven environment.

  • Provide resident and family education to support ongoing care at home.
