LTC / Nursing Homes


Responsibilities

  • Provide licensed practical nursing care to residents in LTC settings.

  • Assess resident conditions and report changes to the RN and care team promptly.

  • Administer medications and document interventions accurately in charts.

  • Develop and follow individualized care plans aligned with resident goals.

  • Monitor and maintain infection control and safety standards on unit.

  • Document all care activities timely and in compliance with regulations.

  • Collaborate with multidisciplinary team to optimize resident outcomes.

  • Engage in performance metrics tracking to meet quality and productivity targets.

  • Maintain on-site presence and contribute to a high-visibility, results-driven environment.

  • Educate residents and families about care plans and treatment expectations.

  • Demonstrate willingness to learn and pursue professional development opportunities.


Experience/Qualifications

  • Active LPN license in good standing with state board.

  • Experience in long-term care or LTC settings preferred.

  • Administer medications and document interventions per policy and regulations.

  • Assess resident conditions and report changes promptly to RN.

  • Compliance with privacy, regulatory, and charting standards.

  • Develop and follow individualized care plans aligned with goals.

  • Strong infection control and safety standards adherence.

  • Accurate documentation of all care activities in charts.

  • Collaborate with multidisciplinary team to optimize outcomes.

  • Engage in performance metrics tracking to meet targets.

  • On-site presence with accountability for individual contribution.

  • Patient/family education about care plans and treatment expectations.

  • Demonstrate willingness to learn and pursue professional development.

  • Contract role with on-site location and ability to meet shift requirements.
