Job Title: Licensed Practical Nurse (LPN) – Long Term Care (LTC)

Job Summary:
We are seeking a compassionate and reliable Licensed Practical Nurse (LPN) to provide quality care to residents in a long-term care setting. The LPN will work under the supervision of an RN or physician to support daily patient needs and ensure a safe, comfortable environment.

Key Responsibilities:

• Provide direct patient care, including monitoring vital signs and administering medications
• Assist residents with daily living activities such as bathing, dressing, and mobility
• Monitor and report changes in patient condition to the healthcare team
• Maintain accurate and timely documentation of patient care
• Support care plans and collaborate with RNs, physicians, and other staff
• Ensure compliance with healthcare regulations and facility policies

Requirements:

• Active LPN license in the state of practice
• BLS certification required
• Previous LTC or skilled nursing experience preferred
• Strong communication and interpersonal skills
• Ability to work in a fast-paced environment

Shift & Schedule:
Day/Night shifts available; weekends and holidays may be required
