LPN/LVN Home Health in Mason, OH with $1760 Weekly Gross

Sage Group HealthCare Staffing is seeking LPN Home Health for a 13 week travel/contract position in Mason, OH. Accepting both local and traveler candidates.

Pay Rate: Range $1600 to $1760 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments

Unit: Home Health

Shifts available: Days

Position Requirements:

• LPN: 2 years - Required
  
• Recent/Current Home Health Nursing experience: 1 year within the last 3 years – Required
  
• Home Care Home Base (HCHB) charting experience – Required 
  
• Wound care, Wound Vac, and Drawing Labs
  
• Must be Covid Vaccinated or provide medical or religious exemption
  

Education, License and Certification Requirements:

• LPN License for the state applying or compact if applicable  - Required
  
• BLS Certification from AHA or American Red Cross - Required
  

Job Description:

• Provide skilled nursing care as ordered by the attending physician under RN's supervision.
  
• Provide all aspects of nursing care for home health patients including but not limited to wound care, wound vac and drawing labs.
  
• Observe and monitor patient conditions and notify RN case manager of any changes in the patient’s condition.
  
• Administer medication as prescribed by the physician under RN's supervision and help decrease re-hospitalizations by educating the patient on preventative measures and making good decisions.
  
• Educate the patient and family on the disease process using teach back methods to ensure patient and family understanding.
  
• Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  
• Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  
• Attend IDT Team meetings
  
• Territories: Mason, OH and Surrounding areas
