Position Summary
A skilled nursing and rehabilitation facility in Honesdale, PA is seeking Licensed Practical Nurses to join its nursing team. The LPN delivers direct resident care under the direction of the RN Supervisor / Director of Nursing, administers medications and treatments, documents accurately, and helps supervise nursing assistants on the unit. This is a hands-on floor role in a resident-centered, survey-ready building.
Key Responsibilities
Administer medications and treatments per physician orders and facility policy; complete accurate MAR/TAR documentation
Perform routine nursing procedures: wound care and dressing changes, catheter care, ostomy care, tube feedings, glucose monitoring, oxygen therapy, nebulizer treatments
Complete admission, readmission, and change-of-condition assessments; report findings promptly to the RN Supervisor or provider
Monitor and document vital signs, intake/output, skin condition, behavior, and response to interventions in the EHR (PointClickCare or comparable)
Supervise and direct CNAs on assigned unit, including assignments, break coverage, and follow-through on care plans
Communicate with residents, families, physicians, pharmacy, and interdisciplinary team members professionally and promptly
Respond to falls, emergencies, and change-of-condition events; initiate protocols and complete incident documentation
Participate in care plan meetings, shift report, and infection control, and support survey readiness at all times
Maintain compliance with PA Department of Health regulations, CMS requirements, HIPAA, and facility policy
