Job Summary:
Tryfacta is seeking a Licensed Practical Nurse for our client in Oneonta NY 13820 . This is a temporary contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title: Licensed Practical Nurse
Location: Oneonta NY 13820
Duration: 13 weeks (Possible extension)
Work Schedule: 8 hrs. Evening and Night

Responsibilities for this position include, but are not limited to:
  • Provide direct nursing care to residents in accordance with physician orders and facility policies.
  • Administer medications, treatments, and nursing interventions safely and accurately.
  • Monitor, assess, and document resident conditions, vital signs, and responses to treatment.
  • Assist residents with activities of daily living (ADLs) as needed.
  • Collaborate with physicians, RNs, CNAs, and other healthcare professionals to develop and implement individualized care plans.
  • Maintain accurate and timely documentation in the electronic medical record system.
  • Observe and report changes in resident conditions to the appropriate healthcare team members.
  • Ensure compliance with infection control, patient safety, and regulatory standards.
  • Provide emotional support and education to residents and families.
  • Participate in shift reports, interdisciplinary meetings, and quality improvement initiatives.

To be considered for this position, you should have: [Skills, Education, or Experience]
  • Active New York State LPN License (Required)
  • Current BLS Certification (Preferred/Required)
  • Minimum 1–2 years of Long-Term Care or Skilled Nursing experience preferred
  • Strong medication administration and patient assessment skills
  • Excellent communication and teamwork abilities
  • Ability to work independently in a fast-paced environment
  • Knowledge of geriatric care and chronic disease management preferred
