LPN / LVN – Long-Term Care (LTC)

Duration: 13 Weeks
Location: 1650 Champlin Avenue, Utica, NY 13502
Unit: Long-Term Care (LTC)
Shift: 2:30 PM – 10:30 PM
Schedule: 5 × 7.5-hour shifts | 37.5 hours/week
Guaranteed Hours: 37.5 hours/week
Weekend Requirement: Every Other Weekend (EOW)
Block Scheduling: Not guaranteed or promised
Local Candidates: Accepted – Local rate applies to candidates residing within 75 miles
EMR: PointClickCare
Scrub Color: Navy Blue

Position Requirements

• Minimum 1 year of recent LTC experience required.

• Travel experience must be clearly documented on the resume.

• Active, unencumbered New York LPN license required.

• Current BLS certification required.

• NURSYS license verification report required, showing active NY LPN license and all active licenses.

• Skills checklist must match the LPN/LTC specialty.

• Valid driver's license with an address matching the candidate's demographic information.

• RTO (Requested Time Off) must be submitted at the time of submission.

• Up-to-date resume required.

  • Multiple Sclerosis (MS)

  • Orthopedic conditions

  • Dementia

  • Memory care patients

Additional Information

• Floating: Required within the same building.

• LTC experience is mandatory.

• Local candidates are eligible; local rate applies to candidates living less than 75 miles from the facility.
