Pay Range: $29 - $31.03 per hour (based on experience)
Schedule:
Monday - Friday 8am-5pm, occasional weekends and on-call rotations

What You Bring (Requirements)

  • Graduate of a school of professional nursing required.
  • Current Colorado Practical Nurses License (L.P.N.).
  • Cardiopulmonary Resuscitation (CPR) certification required.
  • A minimum of one year’s experience in working with the frail or elderly population required.
  • Intravenous Venous (IV) Certification preferred.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.

What You’ll Be Responsible For

The LPN uses and practices basic assessment, participates in planning, executes interventions in accordance with the plan of care and contributes to the evaluation of individualized interventions related to the plan of care. Administers medications, implements primary care orders, and evaluates participant complaints physically and/or by telephone. Provides education to Senior CommUnity Care participant and caregiver in regard to plan of care, nursing treatments or medications. Communicates participant’s condition to primary care givers, physician and interdisciplinary team.

  • Uses and practices basic assessment and assists Clinic Supervisor as designated to complete initial assessment of new PACE enrollee and annual reassessment.
  • Functions as a member of the Interdisciplinary Team. Maintains regular attendance at and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Performs basic nursing services as delegated by Clinic Supervisor.
  • Performs the administration of prescribed medications and treatments in accordance with nursing standards.
  • Maintains participants’ medical record and fulfills Senior CommUnity Care charting and reporting requirements.
  • Triages participants to determine priority of care.
  • Instructs participants and family regarding medications and treatment instructions and care plan.
  • Provides education and training on care and treatment issues of the frail and elderly as requested.
  • Performs other duties as required or requested.
