Summary
This position is primarily responsible for assisting in and providing safe, consistent, and appropriate patient care within the LPN scope of practice. The LPN serves as a patient advocate and focuses on assessing client needs, developing and implementing treatment plans under the guidance of the provider, and providing comprehensive support related to high emergency room utilization and potentially avoidable hospital use.
This role requires strong communication and problem-solving skills, as well as the ability to manage a diverse range of responsibilities. The individual must demonstrate strong computer skills and the ability to work independently.
