Pay Rate
Local: $30 – $35/hr
Traveler: $35 – $40/hr

Position Overview

We are seeking an experienced LPN to provide safe and appropriate nursing care, administer medications and treatments, and monitor patient conditions.

Required Qualifications

• Active Pennsylvania LPN license.

• PA Department of Aging Fingerprint Clearance Letter on file.

• Proven and reliable LPN experience.

• Ability to work the assigned evening shift.

• Ability to complete required Matrix charting and documentation.

• Dependable and able to work according to the facility schedule.

Responsibilities

• Provide safe and appropriate nursing care to residents/patients.

• Perform LPN duties within the scope of practice.

• Monitor patient/resident conditions and report changes appropriately.

• Administer medications and treatments as ordered.

• Complete accurate patient documentation using Matrix.

• Communicate effectively with facility staff and the healthcare team.

• Follow facility policies, safety procedures, and compliance requirements.

• Maintain reliable attendance and provide consistent patient care.
