As a Licensed Practical Nurse (LPN) on our team, you will play a critical role in delivering high-quality clinical care, supporting residents' health and well-being, and ensuring continuity of care through accurate communication, documentation, and collaboration with the healthcare team.

In This Role You Will:

• Provide comprehensive nursing care in accordance with individualized care plans
• Conduct initial and ongoing assessments of residents' health status
• Administer medications and treatments safely and accurately
• Monitor vital signs and report changes promptly to the healthcare team
• Collaborate with physicians and healthcare providers to develop effective care plans
• Coordinate and oversee resident care and services within the nursing department
• Provide health education and support to residents and families
• Promote independence, dignity, respect, and resident safety
• Maintain accurate medical records and documentation in compliance with regulations
• Serve as a resource and mentor to nursing staff and team members

Who You Are:

Required Qualifications

• Graduate of an accredited nursing program.
• Active Licensed Practical Nurse (LPN) license in good standing with the state.
• Basic computer skills and ability to document in electronic systems.
• Ability to communicate effectively both verbally and in writing.
• Ability to provide compassionate, resident-centered care.

Preferred Qualifications

• Bachelor's degree in nursing.
• Prior experience in senior living, long-term care, or healthcare settings.
• Experience working with geriatric populations.
• Experience with electronic health records (EHR) systems.
