Job Summary:


Oneida Health
is seeking a Licensed Practical Nurse (LPN) to provide care for residents at
our Rehabilitation and Extended Care Facility. The LPN will work varying shifts
(day, evening, night) and will collaborate with the Unit Manager, Registered
Nurse (RN), Physician, Certified Nursing Assistants (CNAs), and Ward Clerks to
ensure the daily care needs of residents are met. This role requires excellent
communication skills, sound nursing judgment, and a compassionate approach to
patient care.





Key
Responsibilities:



- Provide compassionate care and treatment to residents
with a customer service attitude.

- Collaborate effectively with the Interdisciplinary
Team (IDT), including Physicians, RNs, UMs, CNAs, and Ward Clerks.

- Use sound nursing judgment to evaluate and observe
changes in resident conditions.

- Communicate quickly and clearly with RNs and
Physicians regarding changes in resident situations.

- Delegate assignments and observe support staff to
ensure appropriate care is provided according to treatment plans.

- Order medications and supplies, prepare equipment,
and assist with wound care as needed.

- Maintain a clean and healthy environment for
residents, ensuring dining areas and personal hygiene spaces are hygienic.

- Schedule resident appointments and notify families
about appointments and transportation.

- Input orders into the computer system, notify the
pharmacy, and maintain accurate record-keeping.

- Attend meetings as directed by the Unit
Manager/Supervisor and participate in care meetings and discharge
planning.

- Ensure compliance with policies and procedures
related to controlled substances and documentation.

- Report building emergencies or concerns promptly and
accurately.

- Report incidents regarding suspected abuse, neglect,
mistreatment, and exploitation.


Qualifications:


Education:



- High School diploma or equivalent.

- Completion of Licensed Practical Nurse course of
study.






Licenses and
Certifications:



- New York LPN.




- BLS/CPR preferred.

Physical
Requirements:



- Ability to lift objects weighing 45 pounds or over.

- Skill in carrying heavy items weighing 45 pounds or
over.

- Demonstrated ability to pull and push objects as
required.

- Capability to reach above shoulder level and perform
tasks requiring hand-eye coordination.

- Proficiency in walking, standing, and sitting for
extended periods during work shifts.

- Capacity to kneel and perform repeated bending
motions as needed.

- Ability to operate motor vehicles.
