As a Nurse Care Manager, you will work in a partial care setting with positions located throughout Southern New Jersey.
Responsibilities:
Provide health education to clients in individual and group setting
Monitor prescriber scheduling with specific attention to provider productivity
Review of lab work and coordination with prescriber, as necessary
Documentation of all nursing activities provided
Support clients through nursing case management supports
Support prescribers with clerical support
Administer medications to include inject-able medications while maintaining precise medication records
Provision of emergency medical care to clients until emergency services arrive, when necessary
Coordination of client care with internal and external providers
Ability to work independently and as part of a team
Other duties as assigned

Qualifications:
High School Diploma or equivalent required;
LPN;
Valid Driver's License.
