Seeking a dedicated Licensed Practical Nurse (LPN) to support individuals with developmental disabilities living in our OPWDD residential settings in Brooklyn. This role is ideal for a compassionate nurse who enjoys hands-on care, building relationships with residents, and working as part of a collaborative interdisciplinary team.
Key Responsibilities
Provide nursing care to individuals residing in OPWDD residential homes
Perform phlebotomy/blood draws and coordinate lab work as needed
Administer medications and treatments in accordance with physician orders and agency protocols
Monitor residents’health status and report changes to the RN or medical team
Maintain accurate documentation and ensure compliance with OPWDD and regulatory standards
Collaborate with residential staff to support each resident’s individualized health care plan
Provide health education and guidance to direct support staff
Qualifications
Active NYS Licensed Practical Nurse (LPN) license
Proven blood draw / phlebotomy experience (required)
Experience working in OPWDD, developmental disabilities, or residential settings strongly preferred
Strong communication and documentation skills
