Essential Duties and Responsibilities

  • Prepare patients for examinations and treatments by obtaining and documenting vital signs, medical histories, chief complaints, and medication information.
  • Assist providers during examinations, procedures, and treatments.
  • Administer medications, immunizations, injections, and treatments as prescribed and within the scope of practice.
  • Perform point-of-care testing, specimen collection, and basic laboratory procedures.
  • Document patient information accurately and timely in the electronic health record (EHR).
  • Coordinate patient care activities, including referrals, follow-up appointments, and patient education.
  • Respond to patient inquiries and triage patient concerns according to established protocols.
  • Assist with chronic disease management and preventive care initiatives.
  • Maintain exam rooms, clinical supplies, and equipment to ensure readiness and compliance with infection control standards.
  • Participate in quality improvement, risk management, and patient safety activities.
  • Ensure compliance with FQHC, state, federal, OSHA, HIPAA, and HRSA requirements.
  • Support care coordination efforts and collaborate with interdisciplinary healthcare team members.
  • Participate in outreach and population health initiatives as needed.
  • Perform other duties as assigned.

Education

  • Graduate of an accredited Practical Nursing program.

Licensure and Certification

  • Current Licensed Practical Nurse (LPN) license in the State of South Carolina.
  • Current Basic Life Support (BLS) certification required.

Experience

  • Minimum one (1) year of nursing experience preferred.
  • Experience in primary care, family practice, community health, or an FQHC setting preferred.
  • Experience working with diverse and underserved populations preferred.

The Licensed Practical Nurse (LPN) provides direct patient care and clinical support services under the supervision of a physician, nurse practitioner, physician assistant, or registered nurse. The LPN plays a vital role in delivering high-quality, patient-centered care within a Federally Qualified Health Center (FQHC) environment, supporting the organization's mission to provide accessible and comprehensive healthcare services to underserved populations.
