Job Requirements
Education Requirements:

• Graduation from an accredited LPN program or equivalent in nursing.

Certifications Requirements:

• Valid LPN license in Texas.
• BLS certification.
• CMAC (Certified MDS Assessment Coordinator) preferred but not required.

Experience Requirements:

• At least 2 years of clinical experience in a healthcare setting, preferably outpatient or clinic-based.
• Experience with mental health populations strongly preferred.
• Prior travel or floating experience is a plus.

Other Requirements:

• Ability to perform on-site interviews.
• Willingness to float between multiple outpatient clinics.
• Strong communication, organizational, and compliance skills.
• Physical ability to work on-site, including potential prolonged standing and patient interactions.

Job Responsibilities

• Provide patient care by performing nursing assessments and establishing individualized care plans.
• Collaborate with multi-disciplinary teams to coordinate treatment and discharge planning for patients with mental health disorders.
