Licensed Practical Nurse (LPN)
**Job Summary:**
The Licensed Practical Nurse (LPN) provides compassionate, patient-centered nursing care under the direction of a Registered Nurse or physician. The LPN administers medications and treatments, monitors patient conditions, performs nursing procedures, documents care, and collaborates with the interdisciplinary healthcare team to promote safe, high-quality patient outcomes.
**Requirements:**
* Graduate of an accredited Practical Nursing program.
* Current Licensed Practical Nurse (LPN) license in the state of practice.
* Current BLS certification preferred or required per facility policy.
* Recent experience in long-term care (LTC), skilled nursing (SNF), rehabilitation, clinic, or acute care preferred.
**Responsibilities:**
* Provide direct nursing care in accordance with individualized care plans.
* Administer medications and treatments as prescribed.
* Monitor, assess, and report changes in patient condition to the Registered Nurse or provider.
* Assist with admissions, transfers, discharges, and ongoing patient care.
* Maintain accurate and timely clinical documentation.
* Collaborate with physicians, nurses, and the interdisciplinary healthcare team.
* Educate patients and families within the scope of practice.
* Follow infection prevention, patient safety, HIPAA, and regulatory standards.
* Perform other duties as assigned.
