LPN – Full Time Evenings (Contract)

Location: New York 14527
Pay: $1,379 – $1,555/week
Schedule: Full Time, 5x8-hour shifts (3:00 PM – 11:30 PM)
Requirements: Every other weekend & holidays required

Position Summary

• Provide direct and indirect nursing care to residents under the supervision of a Registered Nurse

• Deliver age-appropriate care primarily to adult and geriatric populations

• Support a safe, collaborative, and patient-centered care environment

Key Responsibilities

• Collect and report resident physical and psychosocial data to RN and care team

• Assist in developing and updating individualized care plans

• Monitor resident progress and recommend care plan revisions as needed

• Collaborate with interdisciplinary team, residents, and families

• Maintain clinical competency through continuing education and training

• Support staff orientation and mentoring when applicable

• Ensure compliance with safety, infection control, and facility policies

Qualifications

• Active New York LPN License (Required)

• BLS Certification (AHA or RQI only – Required)

• Minimum 1 year LPN experience in a Long-Term Care (LTC) setting

• Experience with adult and geriatric patient populations

• Ability to work in environments with potential exposure to blood/body fluids
