Responsibilities include: Administering medications and treatments according to physician orders and facility policies Monitoring resident health and responding appropriately to changes in condition Making resident rounds to ensure safety, comfort, and quality care Completing required clinical documentation, progress notes, and resident records Supervising and supporting Resident Assistants while fostering accountability and teamwork Communicating resident needs and updates to team members and leadership Assisting with resident assessments, admissions, transfers, discharges, and end-of-life care Receiving and processing physician orders in accordance with facility policies Maintaining infection prevention, safety, and regulatory compliance standards Completing incident and accident reports as required Participating in team meetings and quality improvement efforts Supporting residents and families with compassion, professionalism, and respect Qualifications Current Pennsylvania Licensed Practical Nurse (LPN) License required Strong clinical judgment and decision-making skills Ability to lead and support a team in a positive and respectful manner Commitment to resident-centered care and service excellence Strong communication and organizational skills Ability to remain calm and professional during challenging situations Commitment to Fairmount Homes' mission, values, and faith-based culture Why Fairmount Homes?
