Job Summary:

The LPN partners with residents and the healthcare team to provide safe, patient-centered care in a therapeutic environment. This role includes both direct and indirect care across multiple units.


Position Title: LPN Post Acute / Long-Term Care
Location: Lewiston, ME 04240
Duration: 13 Weeks with possible extension
Work Schedule: Days 12 hrs.
Pay rate: Differ as per the miles.
Within 50 Miles: $46.00/hr.
More than 50 miles: $53/hr. $1,908/week gros s ($684 W2 + $1,224 stipend)

Units

  • Alzheimer's / Dementia Care
  • Transitional Rehabilitation (post-surgery, stroke recovery, etc.)
  • Long-Term Care
Responsibilities for this position include, but are not limited to:
  • Provide direct patient care in LTC and rehab settings
  • Administer medications and maintain accurate documentation
  • Monitor patient conditions and report changes
  • Assist with ADLs and resident support
  • Perform wound care and basic treatments
  • Collaborate with interdisciplinary care teams
  • Float between units as needed

To be considered for this position, you should have: [Skills, Education, or Experience]
  • Active LPN License (ME or Compact)
  • 1 3 years LTC / Post-Acute experience preferred
  • BLS certification required
  • Strong communication and teamwork skills
