Job Summary:
Tryfacta is seeking a LPN Post Acute LTC for our client in Lewiston, ME 04240. This is a temporary 13 weeks contract assignment. If you meet the qualifications listed below and are interested, please Apply Now!

Position Title:LPN Post Acute LTC
Location: Lewiston, ME 04240
Duration: 13 Weeks
Work Schedule: Nights 7p-7a

Pay rate for Local - $40 to $43/hr on W2
For Traveller - $50 to $55/hr Blended


Responsibilities for this position include, but are not limited to:
    • Provide direct and indirect nursing care to residents in a safe, compassionate, and therapeutic environment.
    • Assess, monitor, and document residents' physical, emotional, and psychosocial conditions.
    • Administer medications and treatments as prescribed by physicians and according to facility policies.
    • Develop, implement, and update individualized care plans in collaboration with residents, families, and the interdisciplinary team.
    • Assist residents with activities of daily living (ADLs) and promote maximum independence and comfort.
    • Monitor residents for changes in condition and promptly report concerns to the appropriate healthcare providers.
    • Provide specialized care for residents with Alzheimer's disease and dementia using effective verbal and non-verbal communication techniques.
    • Recognize and appropriately respond to behavioral symptoms associated with dementia, understanding these behaviors as potential forms of communication.
    • Deliver skilled nursing services, including IV therapy, wound care, and geriatric assessments when required.
    • Support rehabilitation goals for short-term patients recovering from surgeries, strokes, fractures, and other illnesses or injuries.
    • Maintain accurate and timely documentation of assessments, treatments, medications, and resident responses.
    • Educate residents and family members regarding health conditions, treatments, medications, and discharge planning.
    • Collaborate effectively with nurses, therapists, physicians, and other healthcare professionals to ensure quality resident care.
    • Adhere to infection prevention, safety, and regulatory standards at all times.
    • Respond appropriately to emergencies and changes in resident status.
    • Participate in shift reports, care conferences, and interdisciplinary team meetings.
    • Float between assigned units and adapt to varying resident care needs as required.
    • Maintain flexibility to provide coverage across all shifts, including weekends, holidays, and a 24/7 care environment.
    • Promote residents' dignity, privacy, rights, and quality of life in a home-like setting.
    • Perform other nursing duties as assigned in accordance with the scope of RN/LPN practice and facility policies.

To be considered for this position, you should have: [Skills, Education, or Experience]
  • Active ME state or Compact LPN License
  • BLS by AHA
  • 2 Years of experience as LPN in SNF/LTC or Acute care
