St. Luke's Hospital Transitional Care Center in Cedar Rapids, IA is currently seeking a dedicated LPN/RN to join our team for 1st shift. This position offers a sign-on bonus for the right candidate who is passionate about providing high-quality care to our patients.
Job Responsibilities:
Providing direct patient care in a transitional care setting under the supervision of a registered nurse or physician
Assessing patients' needs, developing care plans, and implementing treatments
Administering medications and treatments as prescribed
Monitoring and documenting patients' progress and vital signs
Collaborating with interdisciplinary team members to provide comprehensive care
Ensuring compliance with all regulatory requirements and best practices
Qualifications:
Valid LPN or RN license in the state of Iowa
Previous experience in a skilled nursing or transitional care setting preferred
Excellent communication and interpersonal skills
Ability to prioritize and multitask in a fast-paced environment
Strong attention to detail and critical thinking skills
Commitment to providing compassionate and patient-centered care
