Responsibilities:
Provide direct patient care and nursing services to senior residents in accordance with care plans
Administer medications and treatments as prescribed by physicians
Monitor and document patient health status, vital signs, and changes in condition
Collaborate with interdisciplinary care teams to develop and update individualized care plans
Communicate effectively with patients, families, and healthcare providers regarding patient progress
Respond promptly to patient needs and emergencies with professionalism and compassion
Maintain accurate and timely clinical documentation in compliance with regulatory standards
Requirements:
Valid Connecticut LPN or RN license in good standing
Prior experience in geriatric, senior, or long-term care nursing preferred
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills
Ability to work independently as well as collaboratively within a care team
Compassionate, patient-centered approach to senior care
CPR/BLS certification required or ability to obtain upon hire
