Responsibilities

  • Provide licensed practical nursing care to patients in a facility.

  • Monitor vital signs and report deviations to the supervising clinician.

  • Administer prescribed medications and document administration accurately.

  • Identify and escalate potential safety risks or clinical concerns promptly.

  • Assist with patient assessments and care planning under supervision.

  • Maintain meticulous patient records and ensure compliance with privacy standards.

  • Collaborate with the healthcare team to meet patient care targets and outcomes.

  • Educate patients and families on basic care, post-care instructions, and safety.

  • Participate in performance-driven initiatives and track personal metrics against targets.

Experience/Qualifications

  • Practical nursing license (LPN) active and in good standing.

  • Strong patient assessment and care planning under supervision.

  • Excellent clinical documentation and privacy/compliance with health records.
