    Job Description

    We are seeking a compassionate, motivated, and self-driven Licensed Practical Nurse (LPN) who is passionate about serving underserved populations and helping create meaningful change within our community.     Position Summary

    The LPN will play a key role in supporting clinical operations and resident care by assisting with medication management, performing focused nursing assessments, completing delegated wound care tasks, and partnering with staff to promote competency and quality outcomes. We are looking for someone who takes initiative, communicates effectively, and thrives in a collaborative environment.

    Responsibilities

    • Assist with medication management and medication-related documentation
    • Perform focused nursing assessments and report changes in condition
    • Complete delegated wound care and monitor healing progress
    • Collaborate with caregivers and interdisciplinary team members
    • Support staff education, training, and competency development
    • Promote quality care, safety, and dignity for all individuals served
    • Assist with care coordination and follow-up as needed
    • Maintain accurate and timely clinical documentation

     

    Qualifications

    • Current LPN license in good standing
    • Strong assessment and communication skills
    • Ability to work independently and manage priorities effectively
    • Passion for community health and underserved populations
    • Team-oriented mindset with a willingness to teach and support others
    • Experience in long-term care, community health, behavioral health, or assisted living preferred but not required
